High School Student Permission Form

All high school students who are not accompanied by a parent or guardian must email a scanned copy of this completed and signed form before visiting Fondren Library.

• Print this PDF and fill out the form.
• This form must be signed by the student's high school librarian prior to submission.
• Please scan and email the completed form to flsd@rice.edu at least three business days in advance of the scheduled visit for verification.
